When Duplicate Card is not Issued

If any of the documents like the ID proof, residence proof or the FIR copy is not submitted properly by the voter, then he will not be issued a duplicate voter id card.

WHAT IS FORM 001? HOW DO I FILL IT?

Form 001 is the application form required to be filled and submitted by voters to get a photo voter ID card, also called Electors Photo Identity Card (EPIC). There are 2 types of Form 001: Form 001A - For those who are applying for a voter ID card at the same time of applying for voter registration through Form 6/ 8/ 8A. Follow the instructions below to fill out Form 001A: Enter the name of your state and Assembly Constituency(AC) in relevant fields. If you do not know your AC name and AC number, Enter your name, the name of your relation and your address as mentioned in your voter registration form (Form 6 / 8/ 8A) Submit 2 copies of your recent passport sized photographs. Sign and affix one copy to the form (at shown box location) and attach the other copy separately to the form.

Who can file Form-6

First time applicant on attaining age of 18 years or more on the first day of January of the year with reference to which the electoral roll is being revised.

Have you moved to new town, state and you want to change the address in the Voter ID card? Do you want to vote at the new place of shifting? Election Commission allows individual to change the address by filling in the application form called as Form 8 A.

Do you want to change the Name, Age, Father’s/Mother’s/Husband’s name, Sex, Address. Elector’s Photo Identity Card Number? Or do you want to change the photo inside the Voter ID card? Form-8 can be filed for correction to particulars of a person entered in the electoral roll.
